How to Apply for a Police Criminal Record Certificate
A. Required Documents
Foreigners:
a. A completed application form (application forms are available at the application counter or click here to download).
b. The original passport and a photocopy of its data page, or the original Alien Resident Certificate (ARC) and a photocopy of it
ROC Citizens:
a. A completed application form
b. The original National ID Card and a photo copy of it, or the original household certificate and a photo copy of it. If an English version of the certificate is needed, please also present a photocopy of your passport’s data page.
B. Processing Time
It takes two working days to process an application. It may take longer for applications which require an inquiry for criminal record to judicial authorities, such as courts or military courts.
C. Processing Fee
Each certificate costs NT$ 100. Overseas applicants should enclose US$7 in cash, along with a self-addressed envelope while applicants from Hong Kong and Macau should enclose six US dollars in cash. There is an additional fee of NT$26 for postage (optional).
D. Application Procedure
(A)To apply in person:
Applicants may apply at the Foreign Affairs Section of the Hsnichu County Police Bureau during office hours (8:00 to 12:00, 13:00 to 17:00), from Monday to Friday, (except national holidays).
(B)To apply on-line:
1. Please click here to fill out the online application form.
2. After two working days, please take the required documents with you to the Foreign Affairs Section to pick up your certificate.
(C)To apply by mail (for overseas applicants only):
Please send the following documents and enclose US$7 in cash (US$ 6 in cash for applicants from Hong Kong and Macau) and a self-addressed envelope to Foreign Affairs Section, Hsinchu County Police Bureau [No.12, Guangming 6th Rd., Zhubei City, Hsinchu County 302, Taiwan (R.O.C.)]

2. The original passport or the original ARC (you may send a photocopy of your passport or ARC if it has been notarized by R.O.C embassies or consulates abroad).
(D)To have someone apply on your behalf
In addition to the above-mentioned required documents, the person who applys on your behalf also needs to bring an authorization letter and his/her ID and presents the documents to the officer at the application counter.
Notes:
According to the Act governing the Issuance of the Police Criminal Record Certificates, police criminal record certificates will not be issued to applicants who remain on a wanted list or who have been convicted, yet whose sentences have not been served or are being served.
